Fields showing up on preview that aren't on builder

    There shouldn't be two date fields, and the Exhibit A is shown at the bottom when building the form. Also, on the final draft the docusign option is displaying but it is not on the draft form.

    We apologize for inconvenience. I also checked and test your form in the Preview mode in the Form Builder but so far I was not able to reproduce the issue.

    There were no duplicate in the date fields mentioned and the signature field was showing properly as well.

    It could be possible also that it was a temporary glitch in the preview tool.

    Alternatively, you can also just preview the form live using instead its direct URL https://www.jotform.com/form/203361270827149 on any of your browser.

    Previewing the actual form in live mode is more reliable compared to using the Preview option tool inside the Form Builder as it was meant for screening mostly on the design/layout of the form.
