    Hi

We are wishing to design a COVID safe Event Registration form for our members.

We would like to do this from a QR code, have the member input their Name or ID, & have their "personal" information prepopulate the form, prior to their registering for that Event.

Can you advise if / how Jotform could do this, please ?

That's possible through Spreadsheet to Form Widget. The spreadsheet containing the user's data will serve as your database. By entering the ID that is in the spreadsheet, it would populate the fields once the ID matches in the spreadsheet record.

If you need further assistance, let us know.
