I am trying to develop a report based on my customer service satisfaction survey; however when I run the report, regardless if I edit the report to show 5 items, it only shows 3. Please advise.

It looks like that you do not have any answers with selected 'Somewhat Dissatisfied' and 'Not Satisfied' options, that is why the report is not showing these items on the chart.
Also, please note that the Matrix tool is not the best one for the data analysis. It will show combined data for all rows, at this time you can not configure it to show the data per row. Like separate chart for Web and Print Shop.
The solution is to use the Matrix with one row, or switch to the following fields:
- Drop Down
- Radio Button
- Check Box
- Star Rating
