Wordpress Embed - Form does not display on calendar

    Hello our website is wordpress.

    I usually load the iframe code, change the "no" to "yes" in the code and it works in our calendar.  Currently not one code is working in the calendar.  where is has in the past.  has there been changes?

    Hello, 

    From what I can initially see the code seems to be broken, and this may very well be the reason the form is not getting displayed.

    This is the current code on the Calendar:

    It seems to be missing the first part of the code, this is the code from the Embed Form option
    (I've highlighted in yellow the missing pieces):
