Can customers cancel subscriptions in the square dashboard or jotform dashboard or can they only cancel them through the email the receive?
How are each subscription uniquely identified per customer assuming they have multiple subscriptions under the same form? (aka how will the customer know which they want to cancel?)

Hello,
I think you can let your users cancel their subscription by reaching out to you, then you can proceed in canceling your customer'subscription through the Stripe Dashboard as showin on the guide.
However, I think there's also a way for the customers to cancel their own subscription through a Stripe-hosted page that lets your customers manage their subscriptions and billing details: https://stripe.com/docs/billing/subscriptions/customer-portal

You can also view successful transactions on your form submissions page, I believe each payment is identified with a payment ID in which you can access on the dashboard as well:
You can also use the link next to the Stripe Customer ID on email notifications to access specific subscription you want to cancel. See screenshot below as an example:
