I would like to use the Street Name and City in my Email Subjust.

Hi there, I need to use the street name and city to identify the form that was filled out. When I use the "Address" field name it adds too much content.

My email subject is displaying like this:

Street: 111 Pine Street, Street Address Line 2: Suite 1815, : San Francisco, State / Province: CA, Postal / Zip Code: 94111, - Name

I would like it to display it to display like this:

111 Pine Street, San Francisco - Name

Just the street and the city. Some assistance to make this possible would be great. Thanks.
