    OK... https://hipaa.jotform.com/203121342788149

This form had a four part email response set up, pulling different fields from the form and feeding them into the response emails.

You can see a recent four part response received just a few days ago on the screenshot attached.

Now, three of those email response forms are gone from the Settings section and only one remains.

What is going on?? I am the only one with access to the account, and I assure you I did not touch this. In addition even the name of the form is changed, it now shows Therapist--> [email address], whereas I had it set to the client's name.

Please advise and assist me to revert, and don't tell me to go to versions. I am terrified of losing work on the form itself and/or work on the email response forms.

    Hello,

I have checked your form and to clarify, you are referring to the Autoresponder email, correct? I have cloned your form and made a test submission it seems that the form title is the same as shown on the email template:

I have cleared your form cache on our end, to flush minor bugs if there's any. If the issue persists, kindly try to recreate the Autoresponder Email, and let's see if it will make a difference.

Please give it a try and do get back to us if the issue persists. Thank you.

    Hi KrisLei,

I'm not sure you read my whole question. The auto-responder comprised FOUR separate parts.

Now only one remains. instead of Part 1 here, there is supposed to be Part 1, 2, 3, 4.

Just as there always has been, and just as my other forms have.

This is not a minor bug. What happened to the other three parts? I have not touched it.

    Please advise.

    Greetings,

Do you have any coworkers who have access to this form? Usually, the Emails can only be deleted manually.

I believe the form in question is a clone of your form:

You can quickly create the emails again from here.

Should you need further assistance, please let us know.

    Thanks

No, NOBODY has access besides me.

Please revert the form as it was with the four auto-response emails set up. I need this right away. We already lost information today due to Jotform system error.

    Please advise immediately.

Hello! Kindly note that it is not possible for autoresponders to change/delete itself.

As far as I can check, it is not possible to recover the autoresponder email alert templates using the Revision Control tool. You'll need to recreate these email templates.

    Our apologies for the inconvenience.

I'll also suggest you to change your account's password.

    Thank you!

Hello! Kindly check my previous reply. Unfortunately, it won't be possible to revert email alert changes. Only form changes can be reverted with the Revision Control tool.

I didn't ask you to revert, I asked you to indicate when and how this change occurred.

Hello! My apologies for not being clear in my previous reply. It seems that it won't be possible to review or revert email alert changes using the Revision Tool.

But, I've forwarded your request to the concerned department. If they can find it; they'll let you know.

    Thank you!
