How to populate form based on date given?

Hello!

I am creating a form for our live-in caregivers. I would like to be able to fill in a date and then auto-populate to a form based on the date given. Are we able to do that?

Could you elaborate further what kind of populated would you like it to be? For example, if the date is chosen Monday, would you like it to be populated a certain Caregiver Name? Or, would you like to pull data from a spreadsheet?
