The email address you have provided in your last reply is the email address linked to the account you are contacting us from on this ticket.
However, I have checked the mail logs of that email address and found an email alert that was sent from the submission of a form under another account with an email address that shares the same domain as the email address you provided.
The username of the other account is BerkeleyOrtho and it is linked to o***ne****s@ber*****ort*****tics.com.
