Appointment reservation form

I created the Appointment Slot form for the purpose of scheduling time on our Zoom account and with a leader in our group for committee meetings. I need the form to allow scheduling of time slots that will go away once selected.

I need help achieving this. Everything is set up right for it with the exception of the calendar part.
