City, State, Zip Code all on one line

Hi,

I am using the Smart PDF Form Theme. When I insert the Address fields (Street Address, City, State, Zip Code)into my form, I am not able to get the Zip Code on the same line as City, & State. There is plenty of room, and if I try using Shrink, it shrinks all the address fields. I also tried increasing the page width, and it did not affect the layout. Any suggestions on how to get all three fields on one line? I searched for help, and did not find anything that helped.
