I can't remember my log on info but it won't send me reset info to my email from some reason.Page URL:

Upon checking your form, I found out that it is associated with a guest account. This type of account is created when you don't register/sign in to your JotForm account and start building your forms.
The easiest way to resolve your issue is to sign up for a free account using your email address "email@example.com". Your form should automatically be available in your new account. If it doesn't do let us know your account username and we will move the form.
