I created a form (http://form.jotform.ca/form/31975990778274 ) on July 17th of 2013.
When I create it , IT for some reason did not save to "MY FORMS", however the form had been working and is still taking information and compiling into our database via Google Drive. Which is was doing fine until September 7th, 2013.
I had assumed that the reason why the form didn't save to MY FORMS was because we were over our allotte drive space quota.
Basically, I am asking two things:
1) Is it possible to place this form into MY FORMS
2) Can you explain why since September 7th, 2013 the form is only saving people's names and the date of their submission of the form, into our Database?

Hello, upon checking the form URL you gave it does not belong to your account wmmcanada. It belongs to a random guest account. The most probable cause for this is that you have created this form while unknowingly you are currently logged out from your account because of login expiration issues.
Creating forms without signing in will tie those forms to random guest account ID which does not belong to anyone.
Guest accounts have smaller limit numbers than with Free accounts in regards with submissions and storage. This is a possible reason why this form has stopped sending data to your Google Drive as it is already at its limit.
To prevent issues like this in the future, please always sign in and ticking the "Remember me" checkbox before logging in. This will prevent session or log in expirations.
