How do I show hide fields based on the questions?

I need the form below to show certain fields based on the question "Are you a new member or renewing membership?

If a person selects new member, they would need to see all of the fields that are on the form.

If a person selects renewal, they should only see the following fields:

• First name
• Last name
• General contact email
• Select membership type for payment

Please confirm if the Conditions on the form as working they way you wanted it to.

It worked; however, the only question that should be visible on the form should be "Are you a new member or renewing membership?" After a selection is made then it displays the fields based on the selection. At the moment, the question appears but below it is the option to upload your headshot, username, etc.

Workflow for new member

3. Select new member, display all fields.

Workflow for renewing membership
