    Can I pre-populate a series of medical forms?

    I am creating a series of medical forms for patient information then for treatment provided.  In this case, the patient will fill out the initial personal info form, insurance info and medical history forms.  First, because these would be done in sequence, I'd like to transfer basic info, like Name, DOB, etc from the first form to the next ones.  Then I want to create a series of treatment specific forms and pre-populate them with similar or additional info.  For example: A Procedure Form would pull patient name, number, allergies, other info helpful in the procedure to that form from the initial forms??

    Also, is it possible, in say, in a check list, like a list of medical questions (do you have heart disease?, do you have lung disease?, etc), to highlight, as an aler,t a line that is answered in a certain way, as well as pre-populate that alert to a subsequent form? For instance, a patient checks that YES they have an allergy to penicillin.  Then that is highlighted in yellow on the initial form then that alert is automatically inserted into a subsequent form in, say, a tex box or other as a Medical Alert.

    JotForm philosophy is to keep things simple. The scenario you are talking about is an ellaborated one and would require a custom application for you to pull the records from a database

    You could make the forms and use the source code to embed it and customize it for your purposes.. Then, I would suggest to hire a programmer in order to develop the application you are looking for
