Hello,
Yesterday, on January 14th, I received an email from Vitoria Di Iulio at JotForm that was supposed to contain an encrypted copy of a "Consent for Treatment" form but instead I received a copy of a "Studio Rental Agreement." (See screenshot below)
Can someone please correct this?

Thank you for reaching out to JotForm support. It seems that you are contacting us using your JotForm account. May I have your account username or email address associated with the form you mentioned? so we could look into this issue further.
Also, the email tile and email content says "Consent for Treatment", yet the attached PDF in the email you received was actually "Studio Rental Agreement", that was the issue you discovered, correct? We will look into the issues once we checked the form.
If you don't have access to form owner's information, please send us the URL of the form or URL of webpage where the form is embedded. We will be looking forward to hearing back from you with more details.

To answer your question, "the email tile and email content says "Consent

Thank you for the precisions.
I have checked the form and could see that the PDF attached to it was "Studio Rental Agreement"
Changing the PDF attached should solve the issue.

there is no option to choose another form. The Studio Rental form remains
there. And when I choose to activate the PDF attachment, hit Save, and go
back in, it defaults to deactivated.

website, but is attaching the signature to the "Studio Rental Agreement"
which is a form *we have never used*. Perhaps the signature is not syncing
correctly at your end? We followed your advice on how to correct it, but
the problem persists.

You cannot choose another PDF if it hasn't been created first.
Here is how to create a second PDF so you can attach it to the email:

not the issue. The Studio Rental Agreement should not be attached to any of
our forms or signatures, as we have never used or created that form. Please
look further into this issue.

If you do not want to receive the Studio Rental Agreement, you can disable the PDF attachment.
After disabling it, please make sure that you saved the email alert before testing it. We'll wait for your response. Thank you!

Hello Janet - To clarify, is this a cloned form? Or have you used PDF templates when creating the PDF document for this particular form?
I can see the file name of the submission PDF template is 'Studio Rental..." thus providing confusions:
I would recommend deleting the said PDF template and create a new one. Doing so will generate a new PDF document template that has all the fields of the FEE AGREEMENT form.
