I have a site that is not showing up on my google site but the only issue is with Google Chrome. The form is used at my high school and can't be accessed by teachers because its not showing up in Chrome. They can use different browsers but I'd like to have it set up as conveniently as possible.
Here is my site:
If I open it on my phone or at home (off the school network) it opens up fine. How can I fix that on my end (jotform site) to allow acces to the form fields.

We been having the same issue lately, and everything was caused by the changes made by Google Sites. It does not accept non-secure outside content to be placed on any of its gadgets.
To fix that, you will need to re-embed your form using our Secure URL feature. Here's the guide: Setting Up a Secure or SSL Form. And, use the method provided on this thread: http://www.jotform.com/answers/172866-.

Now the form fields have been rearranged on the site. Is that because it's a secure version? It doesn't look the same any more. None of the form fields are missing, they're just not in their normal place.
I looked at the comparison of your form on your website versus being by itself but they both look exactly the same on my end in the latest version of Chrome.
Are you able to possibly provide us with a screenshot of the how you are seeing this rearranged? If you are uncertain of how to do this you can use a free screenshot annoation software such as Jing then just upload the image via our Image Tool. Alternatively, you could also just use a third party image host such as Imgur and then just copy and paste the link or image itself.
