Not able to login to my account

    I am not able to login to my accounts (HIPAA):



    It is stating an invalid password (though I have not changed the password) and also it not sending me any reset instructions (even though I am selecting forgot password and entering my email id). Please help as I have production forms and we need to make immediate changes.

    And now it also stating that I am locked for too many attempts.

    Please help!
