Form not submitting

A customer is saying that they cannot hit submit on the order form below. They hit 'complete order' and nothing happens. Please advise what may be the issue?

Is the form embedded on a website? If yes, please share the website link so we can check it. Also, is there any way we can know if there's an error message when submitting the form?

Please make sure that the user selected/checked these fields before submitting.
