We have been using forms for a long time very successfully, but today one of our forms became scrambled while adding a new field and all conditions stopped working. The form is here: http://form.jotform.us/form/32246710967155
Note that the labels associated with checkboxes and radio buttons now appear INSIDE and OUTSIDE the boxes and the radio buttons. I have been checking the forum for similar occurrences but I have not found any. Can you help?

Thanks for contacting us mmunguia. We will do our best in order to get this problem resolved.
First of all I would recommend you to clone your form and use that cloned form to make changes in order to avoid further issues with your for original form. Use this guide to know how to clone forms: Form Cloning
Then, try removing the fields you added today and save it, it might make the form to work correctly again.
Also, we would appreciate if you share with us the fields you added so that we can clone the form and see if we can make the changes you need without affecting the good functionality of the form.

Thank you. The form is already cloned. The last field added was question #9, at the bottom of the page. I deleted the last 3 fields recently added (Question #9, image at the top of the page and copywrite note at the bottom). The form remains scrambled.

I decided to build the form again from scratch because I could not repair the original one. I left the original on the list, as well as its clone, to review them later.

Can you please check the form again. I've fixed something in your form. I have remove the styling in the Field Labels, in the Free Text HTML field and from the Header.
Let us know if it still not working.

I checked the form you modified and it works fine. If possible, I would like to know what went wrong to avoid repeating the mistake. In the meantime, I built the form from scratch and I will erase the old form and its clone.
The new form I created works well, even with the same styling on the headings and HTML fields.

Thanks for informing us about the good functionality of your form now. I have been checking your fixed form and the one I cloned previously, however it seems I can find what my colleague did to correct your form. So, this message has been forwarded to him and once he gets online I'm sure he will let us kow his solution.
