    Embeded script no longer working

    I have a form that has been displaying just fine for months, even up to a about a week ago, which was the last time it was used for a submission. Now it the embedded code displays nothing.  Even using the "preview" button when I go to grab the code, the popup Test Source Code, it shows nothing below the "Test Embed Code" header.

    If I use the "source" code, the form is displayed, but it doesn't auto-update with changes I make to the form the way the Embed script did before.

    I don't think I changed anything within the form that would disallow displaying on my site.

    The form I am having issues with is titled "GPA MN Hound Relinquishment Form."

    All of my other forms seem to be displaying properly as far as I can tell.

    I just realized the form had accidentally been sent to "disabled" so it was not displaying.  I enabled it and it's fine now.
