When pulling a report, the site location information is not showing up on the spreadsheet. Is there an option I have overlooked?

I checked your report and I was able to see what you are talking about:
However, the same happens with the submissions in the submssion page, there's no information for the Site Locations field:
As you can see disabled the Auto hide empty fields and then the Site Location field appeared with no result, which means that the user didn't enter any information there even when the field is required.
So, having that it mind, it seems that it is an issue related that your required fields are not working correctly. So, I went to test your embeded form in your webpage http://nationalemsacademy.com/spring-info-sessions
And just as I thought, I was able to submit the form even without completing the required fields.
So, it could be that your form script is getting in conflict with some of your webpage script. The best solution for that ckind of issues is to reembed your form using the iFrame code at this time. Please the next guideto know how to get the iframe code: Getting the form iframe code

So, the data for the site locations can not be recovered, correct?

That's correct, the information would not be able to be recovered as the form was enetered without that information.
Definitely embedding the form using the iFrame Method will help you avoid this issue.
