Inbox Error: Missing Completed Form

We received the usual notification in our email about a completed covid form but when I went into our Jotform inbox to retrieve it, there is no form, just something called "Submission 18". Image of the email confirmation and screenshot of the corresponding form in my Jotform inbox attached.
