    I received an email back that I needed to delete credit card information from my form and that your company unlocked my account. I still cannot access the account to make changes to the form!

    Kindly noted that collecting sensitive information such as credit cards through the Short Text widget is violating our Term of Use. We can help you reactivated your account and form if you allow us to remove the offending fields and comply with our Term of Use. Would you allow us and comply to with our Term of Use?
