Header not printing

    When I view the submitted form in the spreadsheet and download the pdf, the header/title of the form doesn't print.

    Hello,

    I tested your form and I can see the header when I tried printing my submission.

    Could you please share a screenshot of the issue? Is that the header (Trio Fast Track Program) you're referring to?
