Form issues

    Hi There

    I am having a serious issue.

    My form as seen online to users, and the form itself and fields all are correct.

    But now that applicants have filled them out, submissions done, the actually submission data is all misaligned, and also using some old form data from another organization I copied the backend out of!


    The form is only supposed be Cochrane Foundaiton, not BRECS. The download PDF versions of submissions are ALL WRONG. They VIEW ok, but download with BRECS old fields.

    I was not able to understand the issue. Just to clarify, someone submitted your form and you receive the wrong copy. Is that correct?

    I checked your form and you're using the Smart PDF Forms. It means that the answer or data should be going to the uploaded PDF. To fix it, please enable the Original PDF Connection to your Online Form option:
