As checked, your account has been suspended as your form: Order Form 1:1 Diet contains plain text fields that collect Credit Card information. It has been tagged as phishing and violates our Terms of Use.
You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
You can use one of the Payment Integrations available with Jotform if you are to collect payments using your forms. Transactions with these integrations are secured instead of using plain text fields.
We can reactivate the account but we need to delete the infringing fields from your form. You must also agree with our Terms of Use.
Please advise.

I am so sorry, didn’t realise that I couldn’t add payment details this way, I apologise, and agree they should be removed
I did try to integrate Square, but it wouldn’t work, the form wouldn’t submit with the Square details on the form, do you know why this is?

I did try to do a Square integration but that didn’t work.
I am sorry for violating the rules, it wasn’t intentional.

I have gone ahead and deleted the offending fields on your form. Your account is active and you should be able to login into your account.
