Duplicated ticket: Notification responses issue

Hi,

I'm having issues with my new forms that I have created.

Responses are not showing up even in autoresponder.

Questions are showing up with no responses.

Notifications is only being sent to my gmail but with no responses.

I transferred this form from Google forms not sure if that is an issue.

Please advise as I really need these to work as set.
