All of the other forms produce a signature, name and date, which line up. The name is further to the right.
On the Records Release the name is further to the left.

May I ask where did you take the screenshot? Is it from a PDF file? I was unable to recreate the issue from my end.
Or are you referring to somewhere else? Kindly let us know.

bertwa1959Replied on February 8, 2021 at 9:11 PMTo be honest, I don't know where it came from. I have counted at least ten ways to save a document. I just know I have a pdf survey that I send to me by sharemail. I open the link and fill out the survey, then hit Enter. I get Thank you. I then (as the company) receive an email. I open it. The form there is definitely not usable. So, I click on Edit all submissions. Downlaod a PDF from there and I get attachment number one above. I am not sure how you are building, sending to customer, receiving back the document which has Edit this submission or Edit all my submissions. (I can do nothing with edit this submission). So I send to the table containing my submissions. From there I select download as pdf. Then I get the above attachment.

From my understanding, it appears the file was downloaded from View Entry within Tables. Is my understanding correct?
Could you kindly try to download it again? It may be temporarily.
