    Hi, I saw that my accounts we’re inactive. I figured it was an oversight or something.. I created another form and here I am.

    We apologize for this inconvenience.

    Your account was suspended because one of your form asks an SSN number. 

    Please be aware that online form builders, including Jotform, are being used for identity theft in the past. That is why SSNs are considered as a Phishing activity as stated in our Terms

    For forms asking this type of information, you need to comply with two things.

    1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

    2. Add a Signature widget where your users sign and agree to provide such sensitive information.

    We would need your permission to delete the SSN field before we can activate your account. Once your account is activated, you will need to add those mandatory field I've mentioned above.

    That's alright. With your permission, I removed the SSN field from your form and activated your account. You can now log in to your account without any issues.
