Also, does it only allow me to select one list for the form? I'd prefer to create a form that allows users to select from several lists.
As I was going through the wizard, the second screen asked me to "Match Form Fields" but didn't provide any apparent way to do that. And it wouldn't let me click next to go on to a separate screen to complete the mapping.
After looking at your Screenshot of the Problem it seems to me like you probably have not configured this correctly between your form and your contant contact account. I think that this guide http://www.jotform.com/blog/51-JotForm-Now-Integrates-with-Constant-Contact will help you out with learning more about our Constant Contact Integration better. It is also important to make sure that you are using the correct named fields and that your list inside your constant contact account are set up appropriately as well so that they can be mapped. Additionally it's important to make sure you've entered in the proper account credentials.

Am I only able to add users to one Constant Contact list? On the form, we allow users to select from three lists that receive different types of emails from us. Right now we're adding them to these lists manually after receiving the Jotform submissions. I'd like to automate this process fully.

Yes, I think that you can have only one list per form with our current Constant Contact integration. It is a great idea to have multiple subscribing options on a single form, so I am forwarding a feature request ticket to our Development Team.
I can not provide any ETA for when it will be implemented, but we will let you know if we have any updates.
