I thought I was filling out a new patient account with Blue Ocean Dermatology. Instead I was led to your site. I gave you personal information that I did not intend to give you Jot Form. Please take my information and remove it from your files. I do not at this time need the services you provide.

If you want the data to be deleted, you need to contact the form owner or the company whose form you have submitted, and they will be able to help you on this.
