    I received a message at login that our account has been suspended by your phishing team. Please advise what we need to do to reverse this action and restore our account.

    The form was found on your account ruthyoung suspicious of phishing activity because you're collecting Passport and information. Collecting this type of data is considered as phishing activity, per our Terms of Use.

    Online Form Builders, including Jotform, are sometimes being used for identity theft, and we go to great lengths to prevent this from happening. 

    If your purpose is not to collect these data from the general public, but from your customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    3. Otherwise, you must enable form encryption on your form if you need to gather critical personal information

    To prevent a new suspension in the future, we had to delete the guilty fields mentioned above. Please follow the recommendations mentioned above to keep the account working.
