    My submission email has a strange header text which is not added by me.

    Hello... I will be adding a "Feedback" form to my CasaDelSol website and I have tried some submissions. They come through perfectly but they display the "Skyline Seamless Gutters" at the top. What is this or have I done something wrong in my design? Below is an example of what I receive. Thanks from Gil McWhirter.

    Did you copy this form?  Looks like it might be in your notifications email possibly?

    Hello CasaDelSol,

    I'm sorry for the trouble caused to you.

    Have you imported/cloned this form? Upon checking your form's notification alert, I found this text to be manually added there. Actually, the JotForm logo was removed and this text was added in that place.

    The most convenient way to solve this problem is to delete your notification and add a new one. When you add a new notification alert, you will see that all question of your form will match with the notification fields and the text "SkyLine Seamless Gutters" will also be removed.
