How do we customize the URL?

Hi there, we have created a form and will upgrade to a plan to eliminate the JotForm branding. But how do we completely customise the URL.

I know we can change the part of it, circled in blue, but what about the rest.

What is the cost of enterprise? I have send sales an email but no response so far.

We don't want any mention of jotform, just want to customise it with our brand - MMS.

There are many other online form that offer this at a reduced rate or part of their monthly subscription.

Can you please let us know?

Please note that it is not possible to remove the JotForm domain form the form URL. Even if you use a custom URL of form, you can only change the part of the form URL which you have displayed in blue.

Alternatively, you can add a web page in your website and then embed your form using your web page URL instead of form URL. When you access your web page, it will display the embedded form.
