I'd like to create a header with our logo and the title of the form in the same line, but I can't figure out how. Please help! Below is a link to what I'd like it to look like, from another form maker.

Thanks for posting your inquiry here in the forum.
While you can easily do that by shrinking the fields - Clicking Cog/Gear Wheel icon > Shrink, it does not currently work for Header fields.
You may try to use the "Free Text(HTML)" tool and doing it in HTML to combine the Header with your logo
And using this code in the HTML. Please adjust accordingly the highlighted parts
<p style="vertical-align:middle"><span style="font-size:24px">Your Title</span></p>
<img src="http://www.abcom-inc.com/accessories/images/square_t1.jpg" alt="" width="300" />
This should create a two column container. At the left is your header and at the right is your logo.
If you need further help, please do not hesitate to ask. Thank you

Hi I was trying to find the place with Free HTML but couldn't find it. Is this functionality for users who have pay membership? Just curious.

Free Text HTML field in under "Form Tools" in our form builder, labeled with just "Text".
If you need help with setting up the codes, let us know but we advise to create a new thread for that. You can use this link to create a new thread: Contact JotForm Support
