Assistance on building form

Hi! I'm not sure if my initial question was sent but I just wanted to check if I could get assistance on building a form. I'm currently stuck on how to make the dropdowns dependent on each other. Also, is it possible to set limits on an option and if so, can that dropdown option be removed once it has reached said limit (ex. if a timeslot has already been reserved, can it be removed from the options for that particular date)?
