Please, reactivate my form!

    Dear Jotform Team!

    I cannot use my form, it says its disabled, and my account is suspended. Please help me, to be able to use it again! I have a small IT repair shop, accepting customers phones computers tablets to repair. Im using the form to send cofirmation to customers, and for myself about the actual repairing if they should leave the unit in my shop. Please help me, its really important!
