Unique Approval Process Workflow: How to create it?

I am trying to create a form for testing. What I need is to be able to have three different proctors administering the test each hour and have a maximum of 5 students sign up for each proctor per hour. The parent would need the option to pick a time to test, and the proctor they want to test with. Is this available? Also is there a simple way for me to see who has signed up for when besides an email?

I have created the flow of activities below. Can you please confirm if it's correct?

1. A sign up form will be sent to parents - parents will pick a time and proctor (Restriction: 5 students per proctor per hour only).
2. Proctors will be notified about their students.
3. On the day of the test, students will receive an email containing the link of the test.
