PDF Version Has Formatting Issues

    Hi,

    I created a form in JotForm and then downloaded it as a PDF. There are some issues with the way questions are formatted on the downloaded PDF (attached). Specifically, "How did you hear about Passage Home?" The responses are not showing and/or are not correctly positioned next to the bubble.

    Similarly, "Are you Under the age of 25 and have children or are pregnant?" The responses are missing.

    Is there any way to fix these problems?

    I have cloned your form for testing and downloaded as a pdf:

    As you can see that the fields are properly aligned.

    Please test it on different browsers/pdf reader and let us know if the problem still persists.
