If you are working with the top link, I do see that the anchored href equals http://jotformpro.com/KEYSTONEMERCH/33075953379970 which currently does not work.
What I suggest is copyin the FormID form the URL: 33075953379970 and pasting it to the Custom URL window, allowing to enable the generated link:
For reference, the Embed Form Wizard is located on the top toolbar on the form editor. By selecting the Setup & Embed form and clicking on the Embed Form icon.
This will allow you to make the form available through that link.

Is it possible to keep both links? I prefer the ID # to be masked by BJsHoliday13 if possible.

Unfortunately that is not possible, when you change from custom url. The one no longer used becomes discarded, and does not work anymore.
An option would be to have 2 copies of the same form. Each with their own custom URL. This would allow you to continue using both custom URLs.
