Credit card info is incorrect

We recently had a fundraiser form and all of the credit card info is incorrect when we download as PDFs. If we download each one individually it is fine, but downloading as a zip file created problems.

Also, we are getting an error message on some computers when logging in that the attempted logins have been maxed and will not log us back in.

Could you please let us know the form in question?
