Your account was autosuspended because your form notification contains restricted keywords that were caught suspicious by our anti-phishing tool. These keywords were commonly used by spammers/phishers to send fraud emails using our system. We have to suspend free accounts doing this without exception. Anyway, after manually reviewing your form, I don't see anything malicious in the email content so I lifted the suspension. I have also added the suspected form notification to our whitelist. You are now good to go.
