My account is suspended for phishing.

I attempted to log into my account and saw that it says that my account was suspended for "phishing". Our organization did not ask any personal questions such as sensitive personal information, such as credit card information, social security numbers, user login credentials, or driver’s license numbers. Please review our forms as soon as possible because our applicants are unable to apply through JotForm, pushing our deadlines further back.
