Hello,
I was in the middle of developing a form, and all of a sudden an error message appeared and I think I lost the form. I logged out, now I cannot log back in using my gmail account. I get "Sign into JotForm with Google" and the button says "Continue as Greg". When I click on the Continue as Greg button, a "sign up now" screen appears (se attached screen shot). I have less than 5 forms in my account (in fact only 2 or 3) so I am unsure why it is forcing me into signing up. Can you please check and fix so that I can access my account and continue developing my forms. Many thanks!

Hello Greg,
You are unable to log in since your account has been suspended.
This is due to the form https://form.jotform.com/210519126657052 which was collecting card information on plain form fields. This is in violation of our Terms and PCI compliance.
I have deleted these fields from the form and activated your account.
