I have set up a custom email to be sent out once info is submitted. That email is not being sent for some reason.

It is possibel to leave the E-mail field empty in the embedded form in the website because it is not set to *required.
If E-mail answer is empty, the Autoresponder E-mail will fail because the recipient address will be missing.
Make it a required field, so that the user will be oblige to provide the e-mail address required.
