tried several times, cleared cache and account's cache...

In order to resolve this issue kindly follow the next steps:
1. Clear your Form Caches, follow this guide: How to clear your form cache
2. Log out Jotform.
3. Clear your browser caches and cookies. How to clean your browser caches and cookies
4. Then Log back in your Jotform account.
