    Thanks for contacting us.

    I have updated your code, please try using this one at this time:

    <script type="text/javascript">
    document.write("<script src=\"https://secure.jotform.us/jsform/32756878592170?referrer=" + window.location.href + "\"></" + "script>");
    </script>

    However, if that does not work, try using the iFrame version at this time, I've done it for you, you can find it at the next link: http://pastiebin.com/528664fa14fca

    Please let us know if yo ustill need assistance with this or kindly open a new thread to post unrelated inquiries.

    Thanks

    The form now shows up but when I sent an email there was no link attatched. I put "refferer" and I can see that in the email but there is no link next to it.

    Thanks for informing us about the continuityu of the issue.

    Could you please share with us the URL where your form is embeded? In that way we will be able to check if there's something wrong with the code while bing embeded in your webpage.

    We'll wait for your reply.

    Thanks

    Vist:

    Once there you should see a Terms of Service. Under that click "Yes" and the form should appear.

    Hello. Thanks for giving us the webpage.

    After investigating, I found out the in hidden field in the form. You have incorrect spelling.

    If you compare that to my colleague's code that he gave you, you can see there is a mismatch in the spelling.

    <script type="text/javascript">
    document.write("<script src=\"https://secure.jotform.us/jsform/32756878592170?referrer=" + window.location.href + "\"></" + "script>");
    </script>

    I already corrected the spelling on your form. Actually, you can put any name as long as the label of the hidden textbox matches the field name in the code.

    Please let us know if this issue persists. Thank you
