Jotforms blank responses

    Patients are completing the COVID screening forms, via Jotform, but the forms are not loading. They are blank.

    Sorry for the inconvenience.

    We had an issue with blank submissions but our developers already fixed it. I clear your form cache on our server. Could you please check and let us know if the issue still persists?
