    Trying to create a Defective Claims form report for multiple items, but not sure how many items?

    We want to create a Dective Claim Report that lists multiple products, but not exactly sure how many items the customer will be reporting on.

    For Example:

    Item #1

    How Many Pieces?

    Invoice #?

    Reason for Defective?

     

    Item #2

    How Many Pieces?

    Invoice #?

    Reason for Defective?

     

    Then repeat the above for about 10 instances and yet we want all of these to be reuired fields. Our problem is, what happens if they only have one item?

    You can create this form by using the technique described on this guide

    How-to-Hide-or-Show-Multiple-Fields-at-Once-Based-On-a-Single-Conditional-Entry

    Here is a sample form http://www.jotformpro.com/form/23088518389971? , notice on this form when you increased the guest number, additional entry becomes available.

    I just need clarification on why all 10 items should be required?

    To my understanding, it will be better to have at least 1 mandatory item, and then if there is more item to claim, the user can just click on an option to add another claim data.

    If you set all 10 to be required then the option to allow the user to provide only 1 item will not work.. because the form requires all 10 item to be fill-in.

    I will wait for your clarification on this.

    The only items that are required would be…
    Of course the first part, Name, Customer Number, Company Name, email address, then
    Item #
    How many items
    Invoice they were billed on
    Reason for claim
    Then it repeat the above 4 if they had another item that they wanted to claim.
